ABSTRACT

Introducción: La hipertensión arterial es una condición médica grave que aumenta significativamente los riesgos de enfermedades cardiovasculares y otras. La mayoría de personas hipertensas ignoran que lo son. Es muy importante medir la presión arterial de manera periódica, es la única forma de detectar la enfermedad. Objetivo: Valorar indicadores de morbilidad por hipertensión arterial y pre hipertensión en el país, a partir de los hallazgos de la prevalencia, tratamiento y control de la enfermedad dados por Encuesta Nacional de Salud, concluida en el año 2019. Métodos: Como parte de la Encuesta Nacional de Salud, Cuba 2018-2019, se realizó un estudio transversal, con el objetivo de identificar la prevalencia de hipertensión arterial, según las variables sociodemográficas, zona geográfica de residencia, en el momento del diagnóstico; el manejo y control de los hipertensos conocidos y la clasificación de los nuevos hipertensos según las cifras sisto/diastólicas presentadas en el momento del diagnóstico. Incluyó a población de 15 años y más, residentes en zonas urbanas y rurales. Resultados: La prevalencia de hipertensión arterial en Cuba fue de 37,3 %, superior en las mujeres con 40,2 %, tanto a nivel global como en las áreas geográficas urbana y rural. Predominaron los hipertensos de ambos sexos en las áreas urbanas. Se incrementó la hipertensión en la medida que lo hizo la edad. Conclusiones: Existió un incremento en la prevalencia total de hipertensión arterial en Cuba en los últimos 24 años. Esta fue superior a la dispensarización reportada a nivel nacional en el primer nivel de atención de salud.


Introduction: High blood pressure is a serious medical condition that significantly increases the risks of cardiovascular and other diseases. Most hypertensive people are unaware that they are. It is very important to measure blood pressure periodically, it is the only way to detect the disease. Objective: To assess indicators of morbidity due to hypertension and pre-hypertension in the country, based on the findings of the prevalence, treatment and control of the disease given by the National Health Survey, concluded in 2019. Methods: As part of the National Health Survey, Cuba 2018-2019, a cross-sectional study was carried out, with the aim of identifying the prevalence of high blood pressure, according to sociodemographic variables, geographic area of ​​residence, at the time of diagnosis; the management and control of known hypertensive patients and the classification of new hypertensive patients according to the systolic/diastolic figures presented at the time of diagnosis. It included the population aged 15 and over, residing in urban and rural areas. Results: The prevalence of arterial hypertension in Cuba was 37.3 %, higher in women with 40.2 %, both globally and in urban and rural geographic areas. Hypertensive patients of both sexes predominated in urban areas. Hypertension increased as age did. Conclusions: There was an increase in the total prevalence of arterial hypertension in Cuba in the last 24 years. This was higher than the dispensarization reported at the national level in the first level of health care.

Resumo Avaliar possíveis diferenças nas trajetórias alimentares de estudantes segundo assiduidade ao Restaurante Universitário (RU) e forma de ingresso na universidade. Experimento natural com graduandos (n=1.131) de uma universidade pública brasileira. Em 2011 e 2013 foram aplicados questionários identificados e autopreenchidos presencialmente sobre consumo regular de alimentos marcadores de alimentação saudável ou não saudável, realização do almoço, jantar e substituição de almoço e/ou jantar por lanche. A variação das práticas alimentares regulares foi avaliada pela trajetória individual de cada estudante obtida pela combinação das respostas nos dois questionários. A análise da associação entre a assiduidade ao RU e a trajetória (positiva ou negativa) foi feita por meio de modelos de regressão logística múltipla. Observou-se associação (IC95% não sobrepostos) entre maior assiduidade ao RU e maior chance de trajetória positiva para realização de jantar e de almoço e para consumo de feijão, hortaliças, hortaliças cruas, frutas, biscoito de pacote, hambúrguer/embutidos e guloseimas e menor chance de trajetória negativa para feijão, hortaliças cruas e salgados fritos. A implementação do RU promoveu significativa melhoria da alimentação dos estudantes assíduos a ele, tanto cotistas quanto não cotistas.


Abstract The scope of this study was to evaluate possible differences in dietary trajectories of students according to their attendance at a University Restaurant (UR) and the basis for their admission to the university. It was an experiment conducted with undergraduate students (n=1,131) from a Brazilian public university. In 2011 and 2013, questionnaires identified and completed in person were applied assessing the regular consumption of foods that were markers of healthy or unhealthy diet, partaking of lunch, dinner and replacement of lunch and/or dinner with a snack. Changes in regular food practices were examined by the individual trajectory of each student versus the studied practices obtained by combining the responses from the questionnaires. The analysis of the association between UR attendance and trajectory was carried out using multiple logistic regression models. An association was observed (95%CI non-overlapping) with greater UR attendance and a higher chance of a positive trajectory for dinner and lunch, and consumption of beans, vegetables, raw vegetables fruit, cookies, packaged salty snacks, hamburgers and candies, and a lower chance of a negative trajectory for beans, raw vegetables, and fried snacks. The implementation of the UR promoted a significant improvement in the diet of conscientious students.

RESUMEN Introducción: Nuevas cepas de SARS-CoV-2 detectadas recientemente provocan aumento de la morbilidad y mortalidad no solo en adultos mayores. Conocer la población vulnerable es un reto. Objetivo: Estimar la prevalencia de adultos vulnerables a la COVID-19 severa y sus factores sociodemográficos asociados. Métodos: Estudio observacional transversal analítico con datos provenientes de la encuesta nacional de salud 2018-2020, que incluye 13 684 personas de 19 y más años de edad. El cuestionario estructurado provee la información de variables sociodemográficas, antecedentes patológicos personales e historia familiar, mediciones de la presión arterial y bioquímicas, entre otras. Vulnerabilidad a COVID-19 severa se construyó según propuesta del CDC de Atlanta, EE. UU y fue la variable dependiente. Resultados: La prevalencia de adultos vulnerables a COVID-19 severa en el país fue de 68,3 %, riesgo superior en el adulto mayor (86,5 %), en los no blancos y en la percepción de condiciones de vida "malas". En adultos mayores ser hombre protege, en tanto, fue riesgo, vivir en la región oriental del país. La hipertensión arterial (39,4 %), seguida del tabaquismo (22,2 %), la obesidad (16,6 %) y las enfermedades respiratorias crónicas (10,6 %) fueron las afecciones médicas más frecuentes. Conclusiones: La vulnerabilidad a la COVID-19 severa en la población adulta cubana es elevada, consecuentemente, constituye un serio problema de salud. Factores no modificables se asocian positivamente a la vulnerabilidad: ser adulto mayor, mujer, color de piel no blanca y padecer hipertensión arterial, en tanto, las "malas" condiciones de vida es el único que puede ser modificado.


ABSTRACT Introduction: New SARS-CoV-2 strains recently detected cause an increase in morbidity and mortality not only in the elderly. Identifying the vulnerable population is a challenge. Objective: To estimate the prevalence of adults vulnerable to severe COVID-19 and its associated sociodemographic factors. Methods: An observational cross-sectional analytic study using data from the 2018-2020 National Health Survey. It included 13 684 individuals aged 19 years and over. The structured questionnaire included information on the sociodemographic variables, personal and family pathological antecedents, and records of blood pressure and biochemistry tests, among others. Vulnerability to severe COVID-19 was developed based on the proposal from the CDC Atlanta, USA and it was the dependable variable. Results: The prevalence of adults vulnerable to severe COVID-19 in the country was 68.3%. At higher risk are the elderly (86.5%), non-white individuals, and those who perceived themselves as having poor living conditions. For the elderly, being male was a protective factor, while living in the western region of the country was a risk factor. High blood pressure (39.4%), smoking (22.2%), obesity (16.6%) and chronic respiratory diseases (10.6%) were the most frequent health problems. Conclusions: Vulnerability to severe COVID-19 in the Cuban adult population is high; consequently, it is considered a serious health problem. Non-modifiable factors are positively associated with vulnerability: being an older adult, female, non-white skin color, and suffering from hypertension; while "poor" living conditions is the only modifiable factor.

RESUMEN Introducción: En Cuba la enfermedad pulmonar obstructiva crónica es sexta causa de mortalidad general. Existe insuficiente conocimiento sobre su magnitud. Objetivo: Determinar la prevalencia y características de la enfermedad en La Habana (2017-2018). Métodos: Estudio multicéntrico transversal, coordinado por Iniciative Burden of Lung Disease, incluyó 349 personas no institucionalizadas de 40 años y más de cuatro consultorios médicos (selección aleatoria). Se aplicó cuestionario con variables sociodemográficas relacionadas con diagnóstico médico previo y tabaquismo. Se realizó estudio de espirometría, pre-pos aplicación de salbutamol para determinar limitación del flujo aéreo y clasificó severidad de la enfermedad en leve, moderada, severa y muy severa. Resultados: Prevalencia global 20,9 % (IC 95 %:16,6-25,2); en hombres 25,3 %, en mujeres 17,7 %. Se incrementó con la edad. Formas leves 60 %, más frecuentes en mujeres 67 % vs. severas en hombres, 22 %. Espirometría previa 15 % de encuestados. Catorce encuestados (19,1 %) tenían el diagnóstico médico previo de enfisema pulmonar, bronquitis crónica o enfermedad pulmonar obstructiva crónica. Prevalencia de tabaquismo 36,0 %, mayor porcentaje en formas severas. La tercera parte (31,7 %), expuestos al humo de tabaco ajeno, en hogar o trabajo; media de exposición diaria 5 h. Conclusiones: La alta prevalencia y subdiagnóstico de enfermedad pulmonar obstructiva crónica la convierten en un importante problema de salud en La Habana. Se pronostica incremento de la prevalencia relacionado con la dinámica poblacional en el país y el alto consumo de tabaco en la población. Se requiere un manejo más integral de esta enfermedad prevenible.


ABSTRACT Introduction: In Cuba, chronic obstructive pulmonary disease is the sixth leading cause of death. There is insufficient knowledge concerning its scope. Objective: To determine the prevalence and characteristics of the disease in Havana (2017-2018). Methods: A multicenter, cross-sectional study coordinated by Iniciative Burden of Lung Disease. It included 349 non-hospitalized people aged 40 and over from four family doctor´s offices (random selection). A questionnaire with sociodemographic variables on previous medical diagnosis and smoking was applied. A spirometric study was conducted before and after salbutamol administration to determine airflow limitations, and the disease was classified as mild, moderate, severe, and very severe. Results: Overall prevalence was 20.9% (95 % CI: 16.6-25.2); with males accounting for 25.3%; and females for 17.7%. It increased with age. Mild forms of the disease affected 60% of the population; they were more common in females (67%) than in males (severe forms, 22%). Previous spirometry was performed in 15% of respondents. Fourteen people (19.1%) had previously been diagnosed with pulmonary emphysema, chronic bronchitis, or chronic obstructive pulmonary disease. Smoking prevalence was 36%, with a higher percentage in the severe forms of the disease. One-third (31.7%) were exposed to second-hand tobacco smoke at home or at work, with a daily average exposure of 5 hours. Conclusions: Chronic obstructive pulmonary disease is a major health problem in Havana due to its high prevalence and underdiagnosis. An increase in prevalence is predicted given the demographic dynamics in the country and the high level of tobacco consumption among the population. Therefore, more comprehensive management of this preventable disease is required.

ABSTRACT

Introduction: Obstetric ultrasound is part of the screening to select the population at high risk of having a congenital malformation. Considering that fetal defects occur in approximately 2-4 out of every 100 live newborns, and are the cause of 35-40% of perinatal mortality in Chile, it is therefore justified to perform the second trimester ultrasound, which presents a high index prenatal screening (56%), with few false positives. Methods: A retrospective, cross-sectional and descriptive study was carried out, by reviewing 6,385 ultrasound scans, which were performed during one year (June 2020-June 2021), at the Regional Hospital of Talca, where 126 fetuses with suspected malformation were detected. Results: Of the total number of patients evaluated, a congenital malformation rate of 1.9% was detected, with cardiac malformations the most frequent, and diabetes mellitus the main risk factor. Conclusions: Antenatal ultrasound study is essential in the first and second trimesters of pregnancy, followed by a referral to an ultrasound committee, emphasizing early and interdisciplinary management. The frequencies found are similar to those reported in the international bibliography

ABSTRACT

INTRODUCCIÓN: Existen evidencias sobre el deterioro del olfato y el gusto en pacientes con la enfermedad por el nuevo coronavirus (COVID-19). Se hallaron diferencias en la proporción de pacientes afectados por estos síntomas en distintas poblaciones. Con base en el patrón de presentación, a partir de abril de 2020, la definición de caso sospechoso en Argentina incorporó la pérdida parcial o total de olfato o de gusto. El objetivo de este estudio fue evaluar la proporción de pacientes con COVID-19 que presentan pérdida de olfato o de gusto en Argentina. MÉTODOS: Estudio multicéntrico. Participaron pacientes con COVID-19 mayores de 18 años confirmados con reacción en cadena de la polimerasa en tiempo real (RT-PCR, por sus siglas en inglés) en muestras obtenidas por hisopado entre el 15 de mayo y el 31 de agosto de 2020. Los participantes completaron una encuesta en línea, que incluyó preguntas sociodemográficas sobre síntomas de COVID-19, comorbilidades y la percepción de la pérdida del olfato y el gusto. RESULTADOS: Participaron 241 pacientes, 79% informaron pérdida de olfato y 54%, de gusto. En 32% de los casos, la pérdida de olfato fue el primer síntoma. La proporción de hombres y mujeres que informaron pérdida olfativa fue similar y lo mismo ocurrió en relación con la pérdida de gusto. Tampoco se observaron diferencias en relación con la edad. DISCUSIÓN: Una proporción elevada de pacientes con COVID-19 presenta disfunción olfativa y gustativa en la población argentina. Resulta importante la disponibilidad amplia de un test para la evaluación del olfato y el gusto con adaptación sociocultural y que pueda realizarse en el hogar. En el futuro, se implementará la realización de un test masivo desechable y digital

ABSTRACT

Introducción: la pandemia del COVID-19 significa un gran impacto en la salud mental de pacientes hospitalizados, ya sea por el aislamiento, la incertidumbre, su propio estado de salud, generando diversas emergencias y urgencias psiquiátricas que deben abordarse. Objetivo: definir la estrategia de intervención del personal de salud para dar una respuesta médica oportuna, eficiente y eficaz, que minimice el impacto que produce el COVID-19 en la salud mental de los pacientes hospitalizados. Método: se realizó una revisión de la literatura sobre el tratamiento de las emergencias psiquiátricas en pacientes hospitalizados por COVID-19, la búsqueda incluyó bases de datos como PubMed, Medscape y Google Académico. Los términos de búsqueda incluyeron SARS-CoV-2, MERSCoV y COVID-19 en combinación con salud mental, tratamiento y farmacología. Resultados: se revisaron artículos sobre alteraciones psiquiátricas de la pandemia por COVID-19 en pacientes hospitalizados y los protocolos que deben implementarse en los casos de urgencias psiquiátricas. Conclusiones: la pandemia por COVID-19 ha generado alteraciones en la salud mental de la población como ansiedad y depresión, principalmente en pacientes hospitalizados, requiriéndose protocolos emergentes en aquellos con patología psiquiátrica previa (AU);


Introduction: the COVID-19 pandemic means a great impact on the mental health of hospitalized patients, either due to isolation, uncertainty, their own health, generating various emergencies and psychiatric urgencies that must be addressed. Objective: to define the intervention strategy of the health personnel to provide a timely, efficient and effective medical response that minimizes the impact that COVID-19 produces on the mental health of hospitalized patients. Method: a literature review was carried out on the treatment of psychiatric emergencies in patients hospitalized for COVID-19, the search included databases such as PubMed, Medscape and Google Scholar. The search terms included SARS-CoV-2, MERSCoV, and COVID-19 in combination with mental health, treatment, and pharmacology.Results: articles on psychiatric disorders of the COVID-19 pandemic in hospitalized patients and the protocols to be implemented in psychiatric emergencies were reviewed. Conclusions: the COVID-19 pandemic has generated alterations in the mental health of the population such as anxiety and depression, mainly in hospitalized patients, requiring emerging protocols in those with previous psychiatric pathology (AU);

Resumen Introducción: Los hongos dematiáceos se caracterizan por la presencia de abundante melanina en su pared celular. Presentan una distribución mundial, siendo más comunes en climas tropicales y subtropicales. Producen infecciones cutáneas y subcutáneas, además de enfermedades alérgicas, neumonías, abscesos cerebrales o infecciones diseminadas. Caso Clínico: Presentamos el caso de un paciente con adenocarcinoma de recto intervenido quirúrgicamente con hallazgo incidental de divertículo de Meckel y en el cual en el estudio anatomopatológico reveló la presencia de un hongo dematiáceo


Introduction: Dematiaceous fungi are characterized by the presence of brown melanine or melanine like pigments in their cell wall. They are generally distributed worldwide, being more common in tropical and subtropical climates. The clinical syndromes are often cutaneous and subcutaneous infections, but can be also responsible of allergic diseases, pneumonias, cerebral abscesses or disseminated infections. Clinical Case: We present the case of a patient with a diagnosis of rectal adenocarcinoma intervening surgically and with an incidental finding of Meckel's Diverticulum. The anatomopathological study revealed the presence of a dematiaceous fungi.

ABSTRACT Objective To describe the food environment of a public university located in the city of Rio de Janeiro, Brazil, and to review the changes that occurred between the years 2011 and 2016. Methods Time trend study (through repeated cross-sectional studies) of the sale of food, culinary preparations and beverages in the University Campus establishments in 2011, 2012 and 2016. Variables regarding the description of the establishments and the offer, price and advertising of food, beverages and culinary preparations were assessed through a checklist. Comparisons between establishments in each year and the analysis of such comparison changes during the period were performed by assessing the difference between absolute and relative values observed in each year. Results Increased number of establishments available, good convenience and financial accessibility were observed. There was a relative disadvantage in the availability of fresh or minimally processed foods and culinary preparations; a predominance of advertising of ultra-processed foods; and lack of nutritional information of culinary preparations. The predominance of establishments selling snacks and candies increased over the years. Conclusion In the period studied, the university food environment did not favor healthy food choices.


RESUMO Objetivo Descrever o ambiente alimentar de uma universidade pública localizada na cidade do Rio de Janeiro e analisar as mudanças nele ocorridas no período de 2011 a 2016. Métodos Estudo de tendência temporal (por meio de estudos transversais repetidos) da comercialização de alimentos, preparações e bebidas nos estabelecimentos existentes dentro do campus em 2011, 2012 e 2016. Foram examinadas, por meio de checklist, variáveis referentes à descrição dos estabelecimentos e a oferta, preço e propaganda de alimentos, bebidas e preparações. A comparação entre os estabelecimentos e a análise das variações verificadas no período foi realizada com base no exame das diferenças entre valores absolutos e relativos observadas em cada ano. Resultados Foram observados aumento da disponibilidade de estabelecimentos, boa comodidade e acessibilidade financeira. Verificou-se desvantagem relativa da disponibilidade de alimentos in natura ou minimamente processados e de preparações, predominância de propaganda de alimentos ultraprocessados e ausência de informações nutricionais das preparações. A predominância de estabelecimentos que comercializavam lanches e guloseimas se acentuou no período. Conclusão No período estudado, o ambiente alimentar universitário não favoreceu escolhas alimentares saudáveis.

Abstract Dietary practices of college students were compared before and after implementation of the University Restaurant (UR) and examined according to frequency of UR use. A natural experiment was conducted with students (n = 1131) of a Brazilian public university using a validated self-completed and identified questionnaire that inquired information on practices of substituting lunch and/or dinner with a snack (≥ 5 days/week) and on regular consumption of foods that were markers of a healthy or unhealthy diet. At the second time point, UR use by students was also assessed based on their attendance to it. Changes in food practices were examined by determining differences in proportions between the two assessments. The analysis of the association between UR use and each dietary practice was carried out using multiple logistic regression models. An association was observed between greater UR use and higher frequency of regular consumption of beans, vegetables, raw vegetables, cooked vegetables and fruit and lower frequency of regular consumption of French fries and/or fried snacks. The UR proved to be an environment that facilitated the adoption of healthy dietary practices and promoted improvement in the diets of the students who were more assiduous to the restaurant.


Resumo Práticas alimentares de universitários foram comparadas antes e após implementação do Restaurante Universitário (RU) e examinadas segundo a assiduidade ao RU. Experimento natural foi conduzido com estudantes (n = 1.131) de uma universidade pública utilizando questionário validado autopreenchido e identificado que abarcou as práticas de substituir o almoço e/ou jantar por lanche (≥ 5 dias/semana) e o consumo regular de alimentos marcadores de alimentação saudável e não saudável. No segundo momento, foi examinada a adesão dos estudantes ao RU por meio de sua assiduidade a ele. A variação das práticas alimentares foi examinada pela diferença entre proporções obtidas nos dois momentos de avaliação. A análise da associação entre a assiduidade ao RU e cada uma das práticas alimentares regulares foi feita por meio de modelos de regressão logística múltipla. Observou-se associação entre maior assiduidade ao RU e maior frequência de consumo regular de feijão, hortaliças, hortaliças cruas, hortaliças cozidas e frutas e menor frequência de consumo regular de batata frita e/ou salgados fritos. A implementação do RU se constituiu como um espaço facilitador da adoção de práticas alimentares saudáveis e promoveu a melhoria da alimentação dos estudantes com maior assiduidade a ele.

RESUMEN Fundamento: los tumores de células granulares son lesiones benignas infrecuentes de crecimiento lento que pueden aparecer en cualquier sitio del organismo. La región de cabeza y cuello incluye las localizaciones más frecuentes (entre el 30 y el 50 % de todos los tumores de este tipo, hasta los del sistema nervioso central). Objetivo: presentar un caso de tumor de células granulares intratiroideo. Caso clínico: se presenta el caso de una paciente femenina de 37 años de edad con enfermedad nodular de la glándula tiroides de crecimiento lento con escasa sintomatología, con confirmación histológica tumor de células de la granulosa. Al examen físico se identificó aumento de volumen en la región anterolateral del cuello, en relación con el lóbulo derecho de la glándula tiroidea, que correspondía con lesión tumoral de 3 cm, mal delimitada, adherida a planos profundos, no dolorosa a la palpación. En la ecografía se observó: lóbulo derecho del tiroides de 4,3 cm x 1,2 cm x 1,4 cm, no homogéneo con lesión nodular de 2,8 x 1,4 cm mal delimitada, de bordes irregulares. Se realizó biopsia transoperatoria que se reportó como positiva de células neoplásicas malignas, sin embargo, el estudio histológico extemporáneo reveló un tumor benigno de células granulares intratiroideo. El tratamiento quirúrgico es curativo en este caso. Conclusiones: el diagnóstico de tumor de células granulares intratiroideo benigno, se presenta con las características de una lesión tumoral maligna, debe tenerse en cuenta en el diagnóstico diferencial de los tumores de la glándula tiroides.


ABSTRACT Background: the granular cells tumors are uncommon benign lesions of slow growth that can appear in any place of the organism. The head and neck region includes the most frequent localizations (between the 30 and 50% of all the tumors of this type, including those of the central nervous system). Objective: to present a case of intra-thyroid granular cell tumor. Clinical case: a case of a 37 year-old female patient with nodular illness of the thyroid of slow growth with scarce symptoms, with histologic confirmation of granular cells tumor is presented. To the physical exam, an increase of volume was identified in the anterolateral region of the neck, in connection with the right lobe of the thyroid gland that corresponded with a tumoral lesion of 3cm approximately, not well defined, stuck to deep planes, not painful to the palpation. In the echography, it was observed: right lobe of the thyroid of 4.3 cm x 1.2cm x 1.4 cm, not homogeneous with a nodular lesion of 2.8 x 1.4 cm not well defined, of irregular borders. During the surgery, a frozen section was examined and reported as positive of malignancy, however, the untimely histologic study revealed a benign intra-thyroid granular cell tumor. The surgical treatment is healing in this case. Conclusions: the diagnosis of intra-thyroid benign granular cell tumor is presented with the characteristics of a malign tumor, it should be kept in mind in the differential diagnosis of the tumors of the thyroid.

Introducción: Aproximadamente, el 15 por ciento de todos los matrimonios no tienen hijos. Según estudios realizados en Cuba, la prevalencia de infertilidad oscila entre 12 y 14 por ciento, destacándose dentro de sus causas los factores pélvicos que afectan las trompas o el endometrio con 30-50 por ciento, seguido de la disfunción ovulatoria con el 10 a 25 por ciento y solo 10 por ciento son de causa desconocida. Objetivo: Caracterizar a las parejas infértiles según variables clínico epidemiológicas seleccionadas. Métodos: Se realizó un estudio observacional, descriptivo, transversal a 224 parejas infértiles, en la provincia Santiago de Cuba. Cuba, en el periodo 2015-2016. Resultados: El 40,3 por ciento de las parejas estudiados presentaron infecciones de transmisión sexual de diferentes etiologías y el 45,6 por ciento se diagnosticaron con infertilidad secundaria. Conclusiones: La edad de la mujer por encima de los 35 años, los antecedentes patológicos personales, la ingestión de medicamentos, la presencia previa de infección de trasmisión sexual así como el antecedente de varios episodios de enfermedad pélvica inflamatoria, están relacionadas con la etiología infecciosa de la infertilidad y pudieran constituir las principales causas de este trastorno en nuestro país(AU)


Introduction: Approximately 15 percent of all marriages do not have any children. According to studies carried out in Cuba, the prevalence of infertility ranges from 12 to 14 percent, among whose highlighted causes are pelvic factors that affect the tubes or the endometrium, accounting for 30-50 percent, followed by ovulatory dysfunction, accounting for 10-25 percent, while only 10 percent are of unknown cause. Objective: To characterize infertile couples based in the selected clinical-epidemiological variables. Methods: An observational, descriptive, cross-sectional study was carried out with 224 infertile couples in Santiago de Cuba Province, Cuba, in the period from 2015 to 2016. Results: 40.3 percent of the studied couples had sexually transmitted infections of different etiologies, while 45.6 percent were diagnosed with secondary infertility. Conclusions: The age of the woman over 35 years, personal pathological antecedents, ingestion of medications, the previous presence of sexually transmitted infections, as well as the history of several episodes of pelvic inflammatory disease, are related to the infectious etiology of infertility and could be the main causes of this disorder in our country(AU)

Introducción: La mortalidad por sobrepeso y obesidad es prevenible, ambas condiciones representan un problema de salud en Cuba. Objetivo: estimar la mortalidad atribuible al sobrepeso-obesidad en la población de 20 años o más. Métodos: estudio descriptivo que utilizó fuentes de datos secundarias para prevalencias de sobrepeso-obesidad y datos de mortalidad por enfermedades no trasmisibles seleccionadas. El impacto del exceso de peso en la mortalidad, se obtuvo calculando las fracciones atribuibles poblacionales a partir de riesgos relativos procedentes de estudios previos. Fue calculada la mortalidad atribuible por causa y sexo. Resultados: del total de muertes (30 656), por los tres grupos de causas seleccionados, 3 785 fueron atribuidas al sobrepeso y la obesidad: 12,3 por ciento. En hombres, 8,1 por ciento; en mujeres 16,8 por ciento. Respecto a las defunciones por todas las causas reportadas en el año para este grupo de edad y sexo, los porcentajes fueron 4 por ciento, 2,4 por ciento y 5,7 por ciento respectivamente. La enfermedad cardiovascular fue la causa más frecuente de mortalidad atribuible: 53 por ciento del total. La segunda causa fue diabetes mellitus tipo II: 25 por ciento y a su vez, la causa específica en la que el exceso de peso tuvo una contribución mayor: 63 por ciento. En hombres la mitad de las defunciones (49 por ciento), se atribuyó al sobrepeso- obesidad; en tanto en las mujeres, en siete de cada diez fallecidas (72 por ciento). Conclusiones: La mortalidad asociada al exceso de peso en Cuba es alta y requiere un enfoque efectivo multisectorial centrado en potenciar oportunidades para un mayor consumo de alimentos nutritivos e incremento de la actividad física(AU)


Introduction: Overweight and obesity mortality is preventable. Objective: to estimate the mortality attributable to overweight-obesity in the Cuban population aged 20 years or older. Method: descriptive study that used secondary data sources for prevalence of overweight and obesity and mortality data from selected non-transmissible diseases: diabetes, cancer and cardiovascular diseases. The impact of excess weight on mortality was obtained by calculating the population attributable fractions from relative risks from previous studies. The attributable mortality by cause and sex was calculated. Results: of the total number of deaths (30 656), 3 785 were attributed to overweight and obesity: 12.3 percent. In men, 8.1 percent; in women, 16.8 percent. Regarding the deaths from all causes reported in the year for this age group and sex, the percentages were 4 percent, 2.4 percent and 5.7 percent, respectively. Cardiovascular disease was the most frequent cause of attributable mortality: 53 percent of the total. The second cause was type II diabetes mellitus: 25 percent and in turn, the specific cause in which excess weight had a greater contribution: 63 percent. In men, half of the deaths (49 percent) were attributed to overweight or obesity; in women, seven out of ten deaths (72 percent). Conclusions: The mortality associated with overweight in Cuba is high and requires an effective multisectorial approach focused on enhancing opportunities for greater consumption of nutritious foods and increased physical activity(AU)

ABSTRACT

El síndrome de cola de caballo (SCC) es una urgencia quirúrgica poco frecuente con una incidencia estimada de hasta 1,8 casos por millón de habitantes, producida por la compresión de las raíces nerviosas en el extremo inferior del canal espinal. La manipulación espinal puede desempeñar un papel etiogénico, provocando la movilización y extrusión del disco. El diagnóstico temprano y el tratamiento oportuno son cruciales, ya que el pronóstico suele ser desfavorable si el tratamiento quirúrgico se retrasa produciendo un daño neurológico permanente. El objetivo de este trabajo es identificar los potenciales factores de riesgo para la manipulación espinal y optimizar esta práctica, evitando así posibles complicaciones derivadas del tratamiento quiropráctico. Presentamos 3 casos de SCC, observados y tratados en nuestro centro, en los que se sugiere una estrecha relación entre la manipulación espinal quiropráctica y la aparición de dicho síndrome. Tras realizarles una RM en la que se observó una hernia discal L5-S1 causante del SCC, los 3 pacientes fueron tratados quirúrgicamente de forma urgente. Los casos presentados demostraron la existencia de una asociación patogénica entre la manipulación espinal y el desarrollo del SCC, al producirse dicho síndrome en las horas siguientes a la manipulación debida a la protusión abrupta de un disco demostrado por RM.


Introduction: Cauda equine syndrome (CES) is a rare surgical emergency with an estimated incidence of up to 1.8 cases per million. It is caused by compression of the nerve roots at the lowest point of the spinal canal. Spinal manipulation can play a pathogenic role, resulting in mobilization and extrusion of the disc. Early diagnosis and timely treatment are crucial, since the prognosis is usually unfavorable and permanent neurological damage likely if surgical treatment is delayed. Objective: The aim of this study was to identify potential risk factors associated with spinal manipulation and, thereby, optimize this practice to reduce the risk of complications from chiropractic treatment. Methods: We present three cases of CES, observed and treated at our center, in which a close relationship between chiropractic spinal manipulation and the appearance of CES was apparent. Results: After magnetic resonance imaging (MRI) revealed an L5-S1 herniated disc causing the SCC, all three patients underwent urgent surgical treatment. Conclusion: The three presented cases demonstrate a strong pathogenic relationship between spinal manipulation and the development of CES, when this syndrome occurs within hours of spinal manipulation, secondary to MRI-documented acute disc protrusion.

ABSTRACT

Existe una asociación epidemiológica entre sífilis e infección por VIH; las úlceras genitales favorecen la transmisión de Treponema pallidum y éste a su vez, la transmisión del VIH. La dermatitis seborreica (DS) se presenta en 2 a 4% de la población general; sin embargo, en los pacientes con infección por VIH/SIDA y SIFILIS es significativamente mayor, llegando a 85% en algunas series. Se presenta el caso de un paciente masculino de 22 años de edad, natural y procedente del Distrito Capital, Venezuela, sin otras patologías conocidas, quien acude a la emergencia por presentar periodos de hetero agresividad y desorientación témporo ­ espacial., El familiar refiere aparición de lesiones descamativas de fondo eritematoso en región facial desde hace 6 meses, no pruriginosas y a la exploración física se evidencia parafasia y pupila de Argyll- Robertson. Se reportan serologías Positivas para HIV por ELISA de 4ta Generación, VDRL Reactivo a 4 diluciones y FTA-ABS Reactivo, La punción Lumbar evidenció pleocitosis, hiperproteinorraquia, hipoglucorraquia y VDRL REACTIVO. La biopsia cutánea con coloración argéntica de Fontana fué positiva para Treponema pallidum, con mejoría clínica significativa posterior al cumplimiento del esquema de Penicilina Cristalina. La dermatitis seborreica puede ser uno de los primeros indicadores de infección por VIH, por lo cual en toda dermatitis seborreica atípica, extensa o que no responda a tratamiento debe solicitarse serología para VIH y VDRL(AU)


There is an epidemiological association between syphilis and HIV infection; genital ulcers facilitatethe transmission of Treponema pallidum and this in turn, the transmission of HIV. Seborrheic dermatitis (SD) occurs in 2 to 4% of the general population; however, in patients with HIV / AIDS and SIFILIS infection it is significantly higher, reaching 85% in some series. We present the case of a male patient, of a 22 years-old , natural from the Caracas, Venezuela, without other known pathologies, who came to the emergency ward due to periods of aggressiveness and temporo - spatial disorientation. Desquamative lesions with an erythematous background in in his face appeared 6 months earlier.The neurological examination showeds paraphasia and Argyll-Robertson´ pupil. Positive serologies for HIV by 4th Generation ELISA and VDRL reactive at 4 dilutions as well as positive FTA-ABS were reported, Lumbar tab evidenced pleocytosis, hyperproteinorrachia, hypoglucorraquia and reagent VDRL . The skin biopsy with Fontana silver coloration was positive for Treponema pallidum and he improved clinically after treatment with a Crystalline Penicillin scheme. Seborrheic dermatitis may be one of the first indicators of HIV infection, so in all atypical seborrheic dermatitis, extensive or unresponsive to treatment, serology for HIV and VDRL should be determined(AU)

ABSTRACT

RESUMEN Las enfermedades no transmisibles (ENT) representan uno de los mayores desafíos para el desarrollo en el siglo XXI, debido al devastador impacto social, económico y de la salud pública que provocan. El propósito de este artículo es describir la evolución y los factores de riesgo de las ENT en Cuba, principalmente en el período 1990-2015, reseñar las acciones emprendidas por el Ministerio de Salud Pública y destacar los desafíos más importantes para su prevención y control. La información contenida proviene de los datos colectados y publicados por la Dirección de Registros Médicos y Estadísticas de Salud, investigaciones sobre los factores de riesgo, otros estudios fundamentados y la documentación de acciones integrales. La mortalidad en Cuba está determinada por cuatro grandes problemas de salud: enfermedades cardiovasculares, tumores malignos, enfermedades crónicas de las vías respiratorias inferiores y diabetes mellitus, que en conjunto causan el 68,0% de los fallecimientos. La tendencia del cáncer es al ascenso y la enfermedad renal crónica emerge como un grave problema de salud. Cuba cuenta con una línea de base conocida sobre los factores de riesgo, de ellos la hipertensión y el consumo de tabaco son los principales relacionados con la mortalidad por ENT. En consonancia con la importancia de estas enfermedades se aprecian hitos e intervenciones de impacto positivo, así como brechas y desafíos en el marco del Plan de Acción Mundial de la Organización Mundial de la Salud para el enfrentamiento a las ENT.


ABSTRACT Non-communicable diseases (NCDs) represent one of the greatest challenges for development of the 21st century due to their devastating social, economic and public health impact. The objective of this article are to describe the evolution and risk factors for NCDs in Cuba, mainly in the period 1990-2015, to outline actions undertaken by the Ministry of Public Health of Cuba, and to highlight the most important challenges with a focus on their prevention and control. The information is based on data collected and published by the Directorate of Medical Records and Health Statistics, research on risk factors, other studies and documentation of comprehensive actions. Mortality in Cuba is determined by four major health problems: cardiovascular diseases, malignant tumors, chronic diseases of the lower respiratory tract and diabetes mellitus, which together cause 68.0% of deaths. Cancer presents a growing trend, and chronic kidney disease emerges as a serious health problem. Cuba has a known baseline on risk factors, and hypertension and tobacco consumption are the main factors related to NCDs mortality. In line with the importance of these diseases, there are milestones and interventions with a positive impact, as well as gaps and challenges within the framework of the World Health Organization's Global Action Plan for the Prevention and Control of NCDs.


RESUMO As doenças não transmissíveis (DNT) representam um dos maiores desafios do século 21 para o desenvolvimento devido ao desvastador impacto social, econômico e de saúde pública que elas causam. O objetivo deste artigo é descrever a evolução e os fatores de risco das doenças não transmissíveis em Cuba, principalmente no período de 1990-2015, para delinear as ações realizadas pelo Ministério da Saúde Pública de Cuba e destacar os desafios mais importantes para sua prevenção e controle. A informação vem dos dados coletados e publicados pela Diretoria de Registros Médicos e Estatísticas de Saúde, pesquisa sobre fatores de risco, outros estudos fundamentados e documentação de ações abrangentes. A mortalidade em Cuba é determinada por quatro principais problemas de saúde: doenças cardiovasculares, tumores malignos, doenças crônicas do trato respiratório inferior e diabetes mellitus, que em conjunto causam 68,0% das mortes. O câncer tem uma tendência crescente, e a doença renal crônica surge como um grave problema de saúde. Cuba tem uma linha de base conhecida sobre os fatores de risco, dos quais a hipertensão e o consumo de tabaco são os principais relacionados à mortalidade por DNT. Em consonância com a importância dessas doenças, há marcos e intervenções com impacto positivo, bem como lacunas e desafios no âmbito do Plano de Ação Mundial da Organização Mundial da Saúde para enfrentar as doenças não transmissíveis.

Fundamento: la incidencia de cáncer renal aumenta a nivel mundial, en específico en Cuba, lo que constituye un serio problema de salud. Debido a esto, asociado al impacto social y económico que tiene la aparición de la enfermedad, se hace necesario caracterizar los pacientes con cáncer de células renales para identificar los factores asociados a su evolución y supervivencia. Objetivo: caracterizar de forma histopatológica el carcinoma de células renales en el Hospital Universitario Manuel Ascunce Domenech. Métodos: se realizó un estudio de tipo descriptivo transversal desde septiembre 2014 hasta junio 2016. El universo estuvo compuesto por 22 pacientes con diagnóstico de cáncer de células renales atendidos en la mencionada institución asistencial y docente, que cumplieron con los criterios de inclusión, con los cuales se realizó la investigación. Resultados: el carcinoma de célula claras resultó predominante (68,2 %) seguido del carcinoma papilar (22,7 %). La mayor parte de los pacientes presentaron tumores entre 10 y 15 cm (68,2 %). Los pacientes con tumores grado III resultaron más frecuentes (45,5 %) seguido de aquellos con grado II (22,7 %). Conclusiones: predominaron los pacientes masculinos entre 55 y 64 años y de piel blanca. El factor asociado más frecuente resultó el tabaquismo. La mayor parte de los pacientes tuvo dolor lumbar, hematuria y tumor palpable como manifestación clínica. El tumor más frecuente fue el carcinoma de células claras con un tamaño entre 10 y 15 cm y estadio grado III.


Background: the incidence of renal cancer increases worldwide and specifically in Cuba, which constitutes a serious health problem. Due to this, associated to the social and economic impact of the onset of the disease, it is necessary to characterize patients with renal cell cancer to identify the factors associated with their evolution and survival. Objective: to characterize histopathologically renal cell carcinoma at Manuel Ascunce Domenech Hospital. Methods: a cross-sectional descriptive study was conducted from September 2014 to June 2016. The universe consisted of 22 patients with renal cell cancer diagnosed at the aforementioned care and teaching institution, who met the inclusion criteria, with whom the research was carried out. Results: clear cell carcinoma was predominant (68,2 %) followed by papillary carcinoma (22,7 %). The majority of patients presented tumors between 10 and 15 cm (68,2 %). Patients with grade III tumors were more frequent (45,5 %) followed by those with grade II tumors (22,7 %). Conclusions: male patients aged between 55 and 64 years and white skin predominated. The most frequent associated factor was smoking. Most of the patients had low back pain, hematuria and palpable tumor as clinical manifestations. The most frequent tumor was clear cell carcinoma, with a size between 10 and 15 cm and stage III.
